Affinity Care Group provides a number of services to support individuals with intellectual and developmental disabilities to live a meaningful life.
We provide Supportive Living services to individuals supported providing 24/7 supports around the clock ensuring that all needs are met. We strongly believe in the person-centered approach and service delivery is based on “What’s important to you as well as what’s important for you.” Supports may include direct assistance as needed with, participating in, and learning how to complete independently activities of daily living (e.g., bathing, dressing, personal hygiene, eating, meal preparation (excluding cost of food), household chores , essential to the health and safety of the person supported, budget management (which shall include supporting the individual in managing his/her personal funds, as appropriate), attending appointments, and interpersonal and social skills building to enable the person supported to live in a home in the community.
We provide Community-Based Day services that support individual’s engagement in community life promoting social interest and involvement based on their needs and preference. Though the name is “Day Services” you can receive these supports any time of day or night, and on any day of the week. Supports provided are implemented to improve skills in the area of self-care, sensory/ motor development, activities of daily living, communication, and social development to motivate individuals and help them to achieve their vision, and execute their action plan. Supports may consist of but not limited to:
- Leisure activities
- Career Exploration
- Educational Activities
- Religious/ Club Memberships
- Volunteer Opportunities
- Maintaining Relationships Friends/Family
We provide supported employment services that support individuals seek employment and work in competitive integrated settings and engage in community life that are not of retirement age, based on each person's needs and preferences Our goal is to create and expand employment opportunities to the people we support and to develop partnerships with workforces in the community. Supported Employment services consist of continuing supports designed to assist individuals for whom it is unlikely that competitive employment at or above the minimum wage will occur without supports, and who, because of their disabilities, need supports to perform in a regular work setting. We provide a job coach/ supervision on--site at the work location.
We provide non-emergency transport to individuals supported to and from approved activities specified in the Individualized Support Plan.
- Transportation to and from medical services covered by the Medicaid State Plan/TennCare Program
- Transportation of school aged children to and from school; and
- Transportation to and from supported or competitive employment; and
- Transportation that is the responsibility of the provider of a residential service
We provide Personal Assistance services that allow individuals to live in their own home / with family and select designated times of the day when supports are needed. We work with an individual’s circle of support to carefully evaluate why, and for what, an individual may require personal assistance. Personal Assistance services focuses on supporting individuals directly. Supports may include of direct assistance as needed with, participating in, and learning how to complete independently activities of daily living (e.g., bathing, dressing, personal hygiene, feeding/assistance with eating, meal preparation excluding cost of food, toileting and incontinence care, assistance with transfer and mobility), household chores essential to the health and safety of the person supported (e.g., washing dishes; personal laundry; general housecleaning in areas of the residence used by the person supported); budget management (which shall include supporting the individual in managing his/her personal funds, as appropriate), supervising and accompanying the person supported to medical appointments if needed, and on personal errands such as grocery shopping, picking up prescriptions, paying bills; and trips to the post office.
We provide In Home Day Services to meet specialized needs related to behavioral challenge, medical concerns/necessity, or other exceptional circumstances. In- Home Day Services is also a possibility for individuals who have selected retirement. Individuals supported are encouraged to engage in therapeutic activities in their home setting.
We provide Respite services to individuals supported when unpaid caregivers are absent or need relief from routine caregiving responsibilities. At times individuals may need to be temporarily relocated from their current living situation or be provided additional services in their current home due to the absence of a supported person’s caregiver for varies reasons including illness, injury or routine relief. We provide services in the supported individual’s home, or in an approved home. We can accompany individuals on short outings for exercise, recreation, shopping or other purposes while providing respite care.
